Clinicato

PSA density and prostate volume calculator

Work out PSA density from the PSA value and prostate volume, or the volume from its ultrasound dimensions with the ellipsoid formula, with the 0.15 threshold.

For doctors and health professionals

A tool to support the clinical decisions of doctors and health professionals, not for self-diagnosis or self-treatment.

This calculator is inside the patient's file in Clinicato: open it from "Medical tools" while you chart the visit, it fills in from the patient's details and vital signs where they are recorded, and adds its result to the visit's plan in one tap, with the "Medical tools" add-on. Try it in your clinic

Calculator

Medical note: The result supports a clinical decision, it does not replace the doctor's judgement or examining the patient, and is to be checked against your reference before any treatment decision.

How it works and when to use it

PSA density = PSA ÷ prostate volume in mL, and the ultrasound volume = length × width × height × 0.52 (ellipsoid formula).

A density above 0.15 ng/mL/mL is linked to a higher chance of clinically significant prostate cancer (Benson et al.), and helps, with the examination and MRI, decide on a biopsy.

Reference: PSA density = PSA ÷ prostate volume; ellipsoid volume = L × W × H × 0.52; 0.15 ng/mL/mL threshold: Benson MC et al. J Urol 1992;147:815-6

Frequently asked questions

Why work out density and not just PSA?

Benign enlargement raises PSA, so its ratio to volume separates the rise expected from size from a more suspicious one.

Does the number alone decide a biopsy?

No, the decision rests on age, examination, family history, MRI and the PSA trend over time.
