How it works and when to use it

One point each for: uncontrolled hypertension, abnormal kidney function, abnormal liver function, prior stroke, prior bleeding or predisposition, labile INR, age over 65, drugs that raise bleeding, and alcohol, for a maximum of 9.

A score of 3 or more means high bleeding risk, to be met by treating the modifiable factors and reviewing more often, not by stopping anticoagulation on the score alone.

Frequently asked questions

Does a high score rule out anticoagulation?

No, in the ESC guidance it is not used alone to withhold treatment, but to correct causes and follow the patient more closely.

Which factors can be changed?

Such as controlling blood pressure, stopping unneeded painkillers and antiplatelets, cutting alcohol and stabilising the INR.
