How it works and when to use it

The score adds points for each risk factor: heart failure, hypertension, age 75 or more (2), diabetes, prior stroke or thromboembolism (2), vascular disease, age 65 to 74, and female sex.

The ESC 2024 guidelines use CHA2DS2-VA without the sex point: oral anticoagulation is recommended from 2 and considered at 1, and the calculator shows both scores.

Frequently asked questions

Why was the sex point dropped in 2024?

Female sex adds risk mainly alongside other factors, and dropping it simplifies the decision without changing much who needs treatment.

Is the score enough to start anticoagulation?

No, the decision weighs stroke risk against bleeding risk and the patient's wishes, with bleeding assessed by a score such as HAS-BLED.
