How it works and when to use it
Each of the five items scores 1 to 3: bilirubin (under 2, 2 to 3, over 3 mg/dL), albumin (over 3.5, 2.8 to 3.5, under 2.8 g/dL), INR (under 1.7, 1.7 to 2.3, over 2.3), ascites and hepatic encephalopathy.
The total runs from 5 to 15: class A 5 to 6, class B 7 to 9 and class C 10 to 15, and in primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC) the bilirubin cut-offs become 4 and 10 mg/dL.

INR or prothrombin time?
The original score used prolongation of the prothrombin time in seconds, current practice uses INR with the cut-offs 1.7 and 2.3.
What is Child-Pugh used for?
To grade the severity and outlook of cirrhosis, to adjust some drug doses as their labels say, and to assess a patient before surgery.
