HOMA-IR calculator: insulin resistance index

Work out the HOMA-IR index of insulin resistance from fasting glucose and fasting insulin, in mg/dL or mmol/L.

For doctors and health professionals

A tool to support the clinical decisions of doctors and health professionals, not for self-diagnosis or self-treatment.

This calculator is inside the patient's file in Clinicato: open it from "Medical tools" while you chart the visit, it fills in from the patient's details and vital signs where they are recorded, and adds its result to the visit's plan in one tap, with the "Medical tools" add-on. Try it in your clinic

Calculator

Medical note: The result supports a clinical decision, it does not replace the doctor's judgement or examining the patient, and is to be checked against your reference before any treatment decision.

How it works and when to use it

Matthews: HOMA-IR = fasting glucose (mmol/L) × fasting insulin (µU/mL) ÷ 22.5, or fasting glucose in mg/dL × insulin ÷ 405.

It is used in research and sometimes in the clinic to estimate insulin resistance, there is no single agreed cut-off for every group, so the calculator gives the number for the doctor to read.

Reference: Matthews DR et al. Diabetologia 1985;28:412-9: HOMA-IR = glucose (mmol/L) × insulin (µU/mL) / 22.5 (= glucose mg/dL × insulin / 405)

Frequently asked questions

Is diabetes diagnosed with HOMA-IR?

No, diabetes is diagnosed by fasting glucose, HbA1c or a glucose tolerance test, HOMA-IR only estimates insulin resistance.

Can it be used on insulin treatment?

No, injected insulin changes the measured insulin, so the result means little.
