GAD-7 anxiety test: a 7-question screening

The GAD-7 anxiety test used in clinics: 7 questions about the last two weeks and a score out of 21 for how severe anxiety symptoms are, a screening, not a diagnosis.

This is a screening test, not a diagnosis, and if you have thoughts of hurting yourself, get help now: call an ambulance (123 in Egypt, 997 in Saudi Arabia, 998 in the UAE) or go to the nearest emergency department.

How it works and when to use it

The Generalized Anxiety Disorder scale (GAD-7) has 7 questions, for each you choose how often it bothered you over the last two weeks: not at all (0), several days (1), more than half the days (2), nearly every day (3), and the calculator adds them up from 0 to 21.

The severity cut-offs are the authors' own (Spitzer and colleagues, 2006): 5 mild, 10 moderate and 15 severe, and a score of 10 or more is worth an assessment by a doctor.

The scale also picks up panic and social anxiety at times, so the doctor decides what the problem is after the interview.

Frequently asked questions

Does this anxiety test diagnose me?

No, it is a screening of how severe the symptoms are, the diagnosis needs an interview with a psychiatrist or psychologist.

What is the difference between everyday worry and an anxiety disorder?

Everyday worry passes and is tied to a situation, an anxiety disorder lasts most days for six months or more, is hard to control and affects daily life, and a doctor tells the two apart.

When should I see a doctor?

If the score is 10 or more, if the symptoms affect your sleep, work or relationships, or if they come with frequent palpitations or breathlessness that need a physical cause ruled out.
